The certified nursing assistants, specifically Ebony, Yvonne, and Tika are doing an excellent job and they should be commended in some sort of way. I’m grateful to be here, to be receiving good food, good care, my meds managed, and a good roof over my head.

When we were searching for rehab/post acute facilities about 2 months ago, reviews of most places were horrible, including Ivy Park. Insurance dictated available facilities and we had to put our faith in the hands of staff. Thankfully, Ivy Park proved to be an OK place for our loved one. It did come with some hurdles and adjustments but at the end of the stay, we were not unhappy. Were things perfect all the time? No. But with communication and tolerance of the circumstances, everything turned out ok. The food was mostly good, there was no overwhelming odors, the community rooms were enjoyable, security was good and therapy sessions were adequate. Transition out of Ivy Park was also smooth.

My husband and I have visited this place multiple times to see family and its been interesting to see how its changed over the last year. While the building is worn and old its starting to look better the staff have been respectful to us and the management is much more receptive than previous owners. The service on the floor4 has been decent – not sure what all the negative reviews are about. There’s definitely rooom for improvement but you should have seen this place a year ago. Oh and the elevators are fine LOL – just old

The staff does an incredible job rehabilitating patients. The therapists and nurses are well-trained. The place is old and could use some touching up, but the staff is making the most of it. They’ve taken good care of loved ones.

Do not send any loved ones here. Promise the world yet this place allowed my father to fall multiple times and then put him diapers making him incontinent. Not just the normal kind of depends he would wear sometimes at night. Like the ones he could pull up and down himself. They put him in some type of fort Knox diaper that I couldn’t even navigate to open. They also had covid and the flu going around and didn’t find it necessary to notify the family or anyone, In fact that there was an outbreak on the floor. I only found out because the janitor came into the room to fix the sink and he mentioned they we’re dealing with an outbreak and sending family home. And then they shove three Patience in one room! I have never been to a facility that has done this. In fact I was told my dad would have his own room And perhaps at most one other roommate. You can believe my surprise when I saw three beds in a room. My father was on a very high dose of steroids and it made him very loopy . The combination of these problems with The exposure to both the flu and covid resulted in his death. There is literally no night staff and the door stays open all night long. The elevator had absolutely no light on it so you were in pitch Black going up. Now over a year later they’re calling me telling me my father owes the money. He was there for roughly 2 days. And this again is where he fell and had to be sent back to the hospital. I was already demanding he be transferred out but he fell once overnight and they didn’t bother to even tell me. The second fall they had no choice but to send him to the hospital. I thought because of the location and this was close to Shay’s side that perhaps it may have been a better alternative. Unfortunately, this place is far from any suitable solution to carrying for a loved one. Steer far away.

Today, nearly the entire leadership team is new, and our focus remains on strengthening quality of care, communication with families, and overall operational standards. While we cannot discuss the details of any resident’s care publicly due to privacy laws, several statements in this review do not accurately reflect the conditions or practices at our facility. Our center follows strict infection control and reporting protocols that are regulated by state and federal authorities. At the time referenced, there was no declared COVID-19 or influenza outbreak on the unit described. Falls unfortunately can occur in healthcare settings, particularly when individuals are experiencing complex medical conditions or medication effects. Our clinical teams implement individualized care plans and safety interventions designed to reduce risk, and any incidents are addressed immediately according to established clinical protocols. Room assignments may vary depending on clinical needs, admissions volume, and regulatory guidelines. We strive to communicate any changes to residents and families whenever possible. Statements regarding staffing levels or facility operations in this review do not reflect the staffing structure required for licensed skilled nursing facilities, which are subject to ongoing regulatory oversight and compliance requirements. Regarding billing, charges are processed through standard administrative and insurance procedures based on services provided during a resident’s stay. Ivy Park has improved by leaps and bounds over the last few months! It’s clear that the new leadership team is making efforts to turn the building around. My interactions with front-line staff have been much more professional and helpful as of late. On the up and up!

First I’d like to know where that nice knowable administrator went. This young boy knows NOTHING about where he works or how things operates. The fact that I had a convo with him when I visited a family member and then followed up with him later in the days, he knew NOTHING. Didn’t remember anything we spoke about. I spoke with the Director of Nursing about the administrator n he informed me that he’s forgetful and has no nursing knowledge. So why is he in control of human beings who has needs?? This is serious running a building with elderly. They are always short staff. Their front desk girl is terrible, no type of friendliness and the admin office is rite there n does NOTHING. What is he there for? Only thing i can think of is to shut the place down because he’s incompetent of his job duties. What is the world coming to that we have no care or respect for our elderly who worked all their lives to be taken care of but instead you got some kid running things but he’s really NOT..I am so frustrated and wouldn’t recommend anyone send their family member to this place!!!

Staffing levels and front desk professionalism are also matters we continuously evaluate and work to improve, as providing a respectful and supportive environment is important to us. Worst place to send someone. Staff is lazy and unprofessional. Patients are uncared for. Door is broke and receptionist is never there to let you in or out so you’re trapped. Staff text and talk on their phone while the patient bell just rings for several minutes. Staff bringing their small kids to work, sitting at front desk once you get off the elevator. Worn down appliances and carpets that need changed. Vent not kicking out heat when it’s 20 degrees outside. Staff taking days to return washed laundry to patients. Doctors not returning calls during business hours.

I think more attention needs to be the patients needs. Like tools or staff to assist patients in stretching there muscles after lying in a bed for a longer period of time. Therapy is only available Monday – Friday. Communication between therapist and pt should be more open. I’ve lived inside my body a long time and try to pay attention to how it feels. So when I say I need to stretch out some muscles 1st, I think I should be heard and listened to. Here you’re left to figure out ways to exercise alone and to hopefully prepare your body to get back to normal. Face it, most of us need help to accomplish this. Not someone telling us you can do this with nothing to grab hold of and at least attempt to do it. A bed is not a piece of exercise equipment..

My sister had the misfortune being sent to this facility. It is a wretched place. The people are ignorant they mistreat the patients. They simply do not care. If you have a loved one never ever send them to this facility.

As a healthcare worker for 35 years this is sickening. If I could give a zero I would You have a patient who is not aloud to walk has no side rails no trapeze suppose to be able to scoot to BSC without any tools for it. Unable to turn side to side therefore has breakdown on skin. Do the nurses even assess the patients? They lie in wet urine no one makes rounds. Food is subpar. No descent padding for beds – use a blanket which is rough on the skin. Minimal personal needs supplies not in reach. Floors need clean. Sounds like they having a party at the nurses station. If it’s a rehab they need to have the tools for the patient to help them move in bed. They need to make rounds every 1-2 hours check on fluid status, bathroom needs. Answer call lights – horrible. This don’t happen overnight. This will be reported to Medicare! . Do your job.

Horrible staffing, let my mother fall 4 times 2nd floor front desk just sits there especially the chubby brown skin girl didn’t catch her name because she blocks it . She heard I was going to turn them in just do your job and you wouldn’t have to worry about that

There’s some nice people, but from my experience from the last five months, I see it as a poorly run facility that should be avoided. There’s minimum effort and poor communication peppered in all departments, and at times it was very frustrating, especially when dealing with the financial side of things. I also agree with a recent review saying that their minimum standards for cleanliness and maintenance should be greatly raised. I don’t do reviews, but the current rating and comments are misleading.

Wouldn’t send my dog here staff is fake smile in you & family’s face literally talk about you the moment you leave the floor or building the state is always in here half the beds don’t work always a shortage on supplies & the smell of urine smacks you in the face soon as you open the doors also if your a person who is always hot bring your own fan or something or else you’ll be so damn hot in this old building oh yea foooodgross wouldn’t feed that to my dog either not all the cnas are terrible either like anywhere else there’s a few good / great ones but this place is TRASH ‼️‼️‼️‼️‼️
